Giliberti Eye and Laser Center participates with most major medical insurance plans. The accepted medical insurances include, but are not limited to:
- AARP Medicare Supplemental Plan
- Horizon Blue Cross Blue Shield
- Medicare (Red, White, and Blue Card)
- Railroad Medicare
- St. Barnabas Healthcare System
For individuals who do not have medical insurance or are covered under an out-of-network plan, we are always happy to see you for an appointment! Please contact your medical insurance carrier for further consultation on coverage and costs for out-of-network services. The office will provide corresponding CPT codes and fees for services rendered.
PLEASE NOTE: All medical insurance co-payments, deductibles, and/or co-insurances are due at the time of your visit. For any individual paying out-of-pocket, we request remittance for office services to be paid in full at the time of your visit. Please contact the office for accepted forms of payment.
